- Where you noticed activity: room, building or outdoor area.
- When it began and how often it appears.
- Clear photographs of the pest or visible evidence, if available.
- Approximate size, shape and color of anything you saw.
- Changes in stored items, food areas, moisture or building access.
- Previous treatments and any areas that are difficult to access.
